IACUC Grant Application Review Form
In accordance with the Public Health Service (PHS) Policy on the Humane Care and Use of Laboratory Animals, no monies will be released by the Office of Research for a PHS or American Heart Association (AHA) award involving the use of live animals for research and/or teaching until the Institutional Animal Care and Use Committee (IACUC) has reviewed and approved each component of the grant proposal that relates to the care and use of animals. In accordance with PHS and AHA policies, any and all experiments involving live animals contained within the grant MUST be approved by the IACUC prior to release of the grant monies.
Upon receiving notice of potential PHS or AHA funding (new or competing renewal) for conducting live animal procedures, complete and submit this form. Submit the "Research Design and Methods" and "Vertebrate Animals" sections of your grant application in the spaces indicated.
Failure to submit the required GAR materials in a timely fashion will delay the release of PHS and AHA funding. In addition, please note that all pending protocols and protocol modifications must be approved by the IACUC before a GAR can be satisfied. Reviews will be processed in the order in which they are received.
